To qualify, please complete the online form. Before approval, you are required to submit a copy of your Retail or Sales Tax Certificate. At this time, files cannot be uploaded with the application. Please send a scanned copy of your certificate to trade@calicocorners.com with the name of the applicant listed in the subject line of the email.
